2025-2026 Education Year Summer Term Mandatory Internship Pre-Evaluation Applications are presented in the attachment.
Students whose internship applications are accepted and those whose applications are partially accepted must complete their deficiencies by May 20, 2026 and submit the original documents to the "Department Secretariat" . Additionally, students whose applications are not accepted can reapply by completing their deficiencies by the specified date (by bringing the original documents).

1-Internship Acceptance Form (STJ-01)(2 Copies)
2-SGK Declaration Form (STJ-02)
3-Undertaking (STJ-03)
4-Health Provision and Activation System (SPAS) Eligibility Certificate
IMPORTANT NOTE: For the SGK transactions of students going to the internship, the Health Provision and Activation System (SPAS) Eligibility Certificate you will obtain by clicking on the https://www.turkiye.gov.tr/spas-mustahaklik-sorgulama link in the e-government application will be submitted to the department secretariat.
