2024-2025 Academic Year Summer Term Mandatory Internship Preliminary Evaluation Applications Are Attached.
Students whose Internship Applications Are Accepted and those with deficiencies will complete their deficiencies between May 01-16, 2025, and submit the originals of the documents to the "Department Secretariat".

1-Internship Acceptance Form (STJ-01)(2 Copies)
2-SGK Declaration Form (STJ-02)
3-Commitment (STJ-03)
4-Health Provision and Activation System (SPAS) Eligibility Document
IMPORTANT NOTE: For SGK transactions of students going to internship, the Health Provision and Activation System (SPAS) Eligibility Document, which you will obtain by clicking on the link https://www.turkiye.gov.tr/spas-mustahaklik-sorgulama in the e-government application, will be submitted to the department secretariat.
